Should This Go to A and E or to a Plastic Surgeon?
When A and E or 999 comes first
Some injuries need A and E, not a private clinic. Go straight there, or call 999, if there is heavy bleeding that will not stop, a deep or large wound, a possible broken bone, a head injury, or anything that worries you a lot.
A hospital emergency department is set up to deal with these quickly and safely, and nothing here should delay that.
When a plastic surgeon is the right choice
In calmer situations, a plastic surgeon is the right choice. If your child has a smaller cut or a facial wound you want closed neatly, or a wound that has already been treated but you would like a better cosmetic result, or an injury from a while ago that healed badly, that is exactly what I help with.
The aim is a careful repair and the smallest scar possible, in a calm, planned setting at a time arranged to suit you.

Every wound leaves some sort of mark, but a careful repair gives the best chance of a faint, well-placed scar that fades over time.
How the wound is cleaned and closed, how soon it is treated, and how it is cared for afterwards all affect the result, which is why a neat first repair matters so much. A small, tidy scar in a good position is a very different thing from a wide or puckered one.
One real advantage of seeing a plastic surgeon is that the same person who repairs the wound also looks after the scar as it heals. If a scar does turn out raised or red later on, there is a clear plan for the scar an injury leaves. What injuries can be repaired, and how?
Cuts, lacerations, bites, soft-tissue wounds and older injuries that healed poorly can often be assessed and repaired by a children’s plastic surgeon.
Most of the injuries I see are cuts and lacerations, often on the face, along with dog and animal bites, soft-tissue wounds, and wounds with grit or glass that needs removing. I also treat the after-effects of burns, and I revise or rebuild injuries that healed poorly some time ago. Hand injuries are handled a little differently, so those are best discussed directly so your child sees the right person.
The repair itself depends on the wound. Small wounds are usually cleaned and closed in clinic under local anaesthetic, which numbs the area so your child feels little. Deeper or more complex wounds are repaired as a day case under a light general anaesthetic, so your child stays comfortable and still, and goes home the same day. Very young children may need a general anaesthetic even for small wounds, so it is always a balance of risk and benefit. After most repairs, the dressing stays on for about a week to ten days, and scar care begins once the wound has closed.

What a repair costs depends on the wound, mainly how deep and where it is, whether it can be closed in clinic or needs day-case surgery and anaesthetic, and whether any reconstruction or scar care follows. Simple repairs under local anaesthetic sit at the lower end, while day-case surgery costs more. The hospital publishes starting-from prices as a guide, and your own quote is based on your child. All care is private and self-pay, with follow-up included.

Should I take my child to A and E or to a plastic surgeon?
Go to A and E or call 999 for a serious injury, such as heavy bleeding, deep wounds, possible broken bones, head injuries, foreign bodies, loss of consciousness or if your child is feeling unwell. Any child with an acute injury must go to A and E first and then after an initial triage, I can get involved. Emergency situations must be dealt with in A and E first. See a plastic surgeon for a smaller cut or facial wound you want repaired neatly, or an injury that has already healed badly.
Will my child’s cut leave a scar?
Every wound leaves some mark, but a careful repair gives the best chance of a small, faint scar that fades with time. How the wound is closed and cared for makes a real difference, and a neat first repair matters.

Dog and animal bites are treated, with careful cleaning and repair to reduce infection and give the neatest result. Bites that are deep, bleeding heavily or on the face should be seen urgently, so go to A and E first if you are worried.

What Matters Most After a Child’s Injury?
Here is the one thing worth knowing. How a wound is repaired the first time does more to decide the final scar than almost anything that follows, so getting a fresh cut seen and closed properly, and early, matters more than any later treatment. Experienced surgeons see this again and again. And if an injury is already long healed, it can still be improved, so it is rarely too late to ask.
